An evidence rated collection of clinical pathway resources for the optimal seniors care

Caremapsforseniors.ca provides health professionals with references to published caremaps in important areas of seniors care. It was developed by a Specialized Geriatric Services team funded by a Sunnybrook & Women's College Health Science Center Innovation Award. The sites development was also supported and is maintained by the Community Education in Aging and Health Project of the Regional Geriatric Program of Toronto. While guidelines provide assistance to individual practitioners, caremaps provide guidance to caregiving teams. We collect caremaps, review and evidence rate each one using a standardized review process. The results of the review process are available to you and the highest rated caremap is made available to you using the menu on the left of the screen. User can also search the entire data base of caremaps that we have reviewed by clicking the "materials reviewed" button also on the left of the screen. We hope that you find this resource useful and we encourage you to comment on any experiences you have had using caremaps.
